If you are applying in person and the person accepting your application knows you, they may sign the bottom of the application form-otherwise, please provide a form of identification such as a drivers licence, birth certificate or passport.
If you are applying by mail, please attach a certified copy of the identification document to the application form-the copy will be destroyed as soon as the organisation is satisfied as to your identity.
Identification requirements for parents applying on behalf of children
In addition to providing proof of identity as per above, parents are also required to provide sufficient proof of their entitlement to make an application on behalf of a child. The provision of one or more of the following kinds of identification documents is also required:
- Identification in the childs name such as immunisation records, current passport, current photo sports club ID, library card; or
- Identification in the names of both the parent and the child such as birth certificate, medicare card or health insurance membership card
It should also be noted that while provision of some of the above forms of identification may provide sufficient proof to enable an application to be accepted, during the processing of a request it may be necessary for the department to make additional enquiries in order to be satisfied that an application is on behalf of a child.

Step 3
If you are required to pay an application or processing fee, include your cheque, money order or receipt number if you have paid a Receiver of Territory Monies.

Step 4
Information Services will acknowledge your application upon receipt and contact you if further information is required.
From the date Information Services receives your application, the department has 30 days in which to respond, or will be considered to have refused access. If necessary, an extension of time may be negotiated with you.
